1095-B and 1095-C Forms

Requesting Forms 1095-B and 1095-C

In accordance with IRS Notice 2025-15, Port Washington School District will provide copies of Forms 1095-B and 1095-C only upon request. Employees who require a copy of their form should email [email protected] to submit their request.

Forms will be provided within 30 days of the request or by January 31 of the following year, whichever is earlier.
