    New York State law prohibits the taking of any medication by students during school hours unless the school nurse has been furnished with the following information:

     

     1.  Written request from parent

          2.   Doctor’s note stating:

    -         Name of Prescription

    -         Dosage and time to be given, and number of days

    -         Reason for taking medication

           3.  Medication properly labeled in a bottle from the pharmacy

     

    These requirements apply to Tylenol, Advil, cough medicine and any other over-the-counter medicine, as well as prescription medication.

     

    All medication will be stored in the Health Office and dispensed only by the School Nurse according to written instruction only.

     

    A medication authorization form may be obtained from the Nurse’s Office and must be renewed at the beginning of each year or when medication is changed.

     
     
    Please keep all our children safe by not giving your child medication to hold in their pocket.
